“New Vendor Packet”.

W-9 FORM or AWAITING TAX ID NUMBER (T.I.N.) CERTIFICATION if applicable
DECLARATION OF VEHICLES FORM
CERTIFICATE OF LIABILITY INSURANCE
Contractor/subcontractor provides COPY OF WORKERS’ COMPENSATION CERTIFICATE
